Healthy communities
The Early and Periodic Screening Diagnosis and Treatment program (EPSDT) is a special health care program within Medicaid/CHP+. EPSDT outreach and case management is available free of charge to pregnant women and children who are under the age of 20 years old.
The EPSDT outreach coordinator can help:
-Assist people with the Medicaid/CHP+ application process
-Explain Medicaid/CHP+ benefits
-Find available Medicaid/CHP+ providers (doctors, dentists, vision care, mental health)
-Encourage families to seek preventive and appropriate health care

How it works:
The Garfield County outreach worker contacts Medicaid/CHP+ families to explain medical, dental, and vision benefits and encourages families to use these services. Outreach workers can also connect families to available low cost or no cost services and make referrals to places in the community like food banks, housing agencies, Head Start and WIC.
