Children with special health care needs


HCP – Health Care Program for Children with Special Needs

Free. For any child ages 0-21 years who may have a special health care need

Children and youth with special health care needs are defined as "... those who have or are at increased risk for a chronic physical, developmental, behavioral, or emotional condition and who also require health and related services beyond that required by children generally."

HCP’s goal is to assess the needs of the special needs child and their family. The program coordinator will help to support the family in meeting these needs and help to improve the coordination of needed care.
