Self-Sufficiency Assistance

Mission statement
To provide services and support and to assist in obtaining and maintaining an improved quality of life for eligible residents.

Vision statement
All eligible individuals will receive information and assistance from available resources.

Our office is open to the public Monday through Friday from 9 am to 4 pm. To apply for assistance, pick up an application from either our Glenwood Springs office or our Rifle office, call to make other arrangements to obtain an application, or visit Colorado Department of Human Services.

Colorado citizens can now screen for eligibility, apply for benefits, submit household changes and check the status of their benefits on the State’s new PEAK site at www.colorado.gov/benefits.

Our public assistance programs are:

  • Food Assistance Program
  • Colorado Works/TANF Program
  • Medicaid
  • Old Age Pension (OAP)
  • Medicare Savings Program
  • Aid to the Needy Disabled/Blind (AND)
  • Child Care Assistance Program (CCAP)
  • Low Income Energy Assistance (LEAP)

Walk-in interviews are conducted Monday through Friday from 9:00am to 3:00pm.

We offer a variety of programs and services. Each program has its own guidelines for eligibility; please refer to the chart below:

This chart is based on your monthly gross income, which is ALL earnings BEFORE taxes PLUS all unearned income in the household (Child Support, SSI, etc.)

Each program also has resource limits. Amounts are subject to change.

Each program requires certain forms of verification. The verification that will or may be requested is listed below:

Verification of citizen and identity:
We must see the original document.

  • Passport
  • State issued birth certificates
  • Identity – driver's license, school id, government issued id, affidavit

Other verification that may be requested:

  • Identification for head of household
  • Social security number or card
  • Proof of residency
  • Income (earned/unearned)
  • Alien Status
  • Resources
  • Rent/mortgage receipts
  • Utility expenses
  • Phone bills
  • Child support expenses dependent care expenses
  • Medical expenses if you are elderly or disabled
  • Verification of pregnancy with due date
  • Birth certificate
  • Marriage license
  • Divorce decree
  • Medicare card
  • Life insurance policies
  • Burial policies
  • Children's immunization records
  • Child support paperwork
