Mission Statement:  To provide services and support and to assist in obtaining and maintaining an improved quality of life for eligible residents.
Vision Statement:   All eligible individuals will receive information and assistance from available resources.

Our office is open to the public Monday through Friday from 9:00 a.m. to 4:00 p.m.  To apply for assistance you can pick up an application from either our Glenwood Springs office or our Rifle office, call to make other arrangements to obtain an application or go online at www.cdhs.state.co.us

Potential clients have the ability to check if they may be eligible for the various Public Assistance programs below and existing clients may check their benefits on the State's new Peak site at www.colorado.gov/benefits

Our Public Assistance programs are:

Walk-in interviews are conducted Monday through Friday from 9:00am to 3:00pm.

We offer a variety of programs and services.  Each program has its own guidelines for eligibility; please refer to the chart below:

Each program requires certain forms of verification.  The verification that will or may be requested is listed below:
